Parents are required to attend the parent meeting for each sport their child plays.
All participating students must have a valid Pre-Participation Physical Examination form on file with the school at the time of registration. If you already completed and returned the Pre-Participation Physical Examination form earlier this summer as part of the Required Forms process, you do not need to submit the form again.
Sports registration forms for 2023-2024 are available on FACTS Family Portal under the Resource Documents tab.
